If you select to purchase an Alpha-Stim device at the VAT-free price, we will require you to complete the VAT declaration below, before we can dispatch your goods.
We will also take your selection of the VAT-free pricing option as your electronic signature, declaring and confirming your eligibility to complete your purchase free of VAT.
Please be aware that Blackhill Health is unable to give any advice regarding the eligibility for VAT relief. If you are in any doubt as to whether you are eligible to receive goods or services zero-rated for VAT you should consult HM Government "Notice 701/7 VAT reliefs for disabled people" (which you may search for online) before placing your order.
It is the customer's responsibility to ensure their eligibility for this exemption and you may be held liable for making false declarations in this regard.
We are obliged to provide the UK tax authorities, on demand, the details of customers who claimed VAT relief on the purchase of an Alpha-Stim device.
